Become a Patient

We would love to have you as a patient here at New Hope Clinic! Before you can schedule an appointment, you have to make sure you are eligible to receive our services. To be eligible, you have to meet these criteria:

Charity Care Program (For Uninsured)

  • Brunswick County Resident
  • Age 18 or older
  • Household income below 300% of Federal Poverty Level
  • Uninsured and not eligible for Medicare, full VA Medical Benefits, private insurance, or affordable employer insurance (premium <9% of gross household income). *Applicants with Family Planning Only Medicaid may qualify.

OR

Adult Medicaid Program (*Currently no dental services available for patients with Medicaid)

  • Brunswick County Resident
  • Age 18 or older
  • Have Medicaid and no other insurance

 

How to Start

Adult Medicaid Program

  • We accept the following NC Medicaid plans: *Must not have additional private insurance or Medicare
    • AmeriHealth Caritas
    • Carolina Complete Health
    • HealthyBlue
    • NC Mediaid Direct
    • Trillium Health Resources
    • United Healthcare Community Plan
    • WellCare
  • Call 910-845-5333 to verify your Medicaid eligibility and schedule an appointment (*Currently no dental services available for patients with Medicaid)

OR

Charity Care Program (for Uninsured)

  • The NHC (New Hope Clinic) Charity Care Program helps uninsured individuals access healthcare they need, including services through hospital financial assistance programs.
    • Application Process
      1. Complete and sign the eligibility application packet
        • Application Packet – English
        • Application Packet – Spanish
        • No printer? No problem!
          • Pick up a packet at:
            • 201 W Boiling Spring Rd, Southport, NC 28461
            • 3610 Express Dr, Shallotte, NC 28470
          • Request a packet by mailed to you by calling 910-845-5333
      2. Attach the required documents
        • Proof of Identity (provide 1 of the following)
          • Photo ID (e.g. state-issued ID, driver’s license)
          • Permanent Resident Card (Green Card)
          • Passport
        • Proofs of Residence (provide 2 documents showing your full name and current address, dated within the last 90 days). Examples:
          • Unexpired photo ID with your current address
          • Utility bill
          • Lease agreement
          • Bank statments
          • Tax return
          • Paystub
          • Phone bill
          • Benefits award or denial letter
          • Any government-issued statement
        • Proofs of Income (Provide all documents that apply to your household situation. Must be most recent.)
          • One (1) month of recent paystubs for each job
          • Two (2) months of self-employment records
          • Two (2) months of recent bank statements (all accounts, all pages)
          • Current Social Security or Disability Benefits Letter
          • Current Unemployment Benefits Letter
          • Documents showing any other income (e.g. rental income, gifts from family)
          • Letter of Support (if no income)
          • Last year’s Tax Return – Form 1040 and Schedules or IRS Form 4506-T(for verification of non-filing)
        • Proofs of Uninsured Status (Provide any of the following, if applicable. We will help you figure out what is needed and how to get the letters.)
          • VA Benefits Denial Letter
          • Medicaid Denial Letter
          • Medicaid Family Planning only approval letter
          • Employer Insurance Verification Form
          • Private Insurance Cancellation/Termination Letter
      3. Submit your completed application and documents in one of these ways:
        • Drop off or mail to: 201 W Boiling Spring Rd, Southport, NC 28461
        • Drop off to the dropbox outside: The Lord’s Food Pantry, 3610 Express Dr, Shallotte, NC
        • Email to: info@newhopeclinicnc.org
        • Fax to: 910-845-5366
      4. Wait for your application to be reviewed:
        • We will notify you if we need more information
        • If you do not hear anything in one week, call or email
        • You will be informed of the final decision (approved or denied) by phone and/or mail.
          • If approved – You will receive coverage for 90 days or 1 year, and we will schedule your appointment over the phone. You must notify the Clinic within 7 days if your eligibilty criteria changes.
          • If denied – we will provide a list of other resources and assist you in connecting you with alternative services

          • Please note, walk-ins are welcome, but there may be a wait time
